Birthday Party Request Form

Please allow two weeks prior to date for staffing accommodations.
If you have any general special requests, please list them here. We will do our best to accommodate requests per our guidelines.
I agree to pay the non-refundable deposit of $150 of total dues today to reserve my party date. I agree to pay the remaining balance prior to the start of my party. Party dues are non-refundable; if I need to reschedule, I understand that I may do so with at least 24 hours notice. If I cancel with less than 24 hours notice or fail to present at my scheduled party time, I will be charged the full party fee. I understand and agree to all party terms stated in the party brochure. I understand that all party participants must wear proper footwear. Please no flip flops, sandals, or open-toed shoes (Pool parties are an exception to this policy).
